Foundations of Psychedelic Psychotherapy

Thank you for your application to the Foundations of Psychedelic Psychotherapy program at the Michener Institute of Education at UHN hosted in conjunction with the UHN Psychedelic Psychotherapy Research Group. By submitting this application, you understand and agree to the following:

· I understand that this program is demanding in nature and will involve a total of approximately 100 hours. Each week I will be expected to review 1-3 hours of recorded lectures and attend a live two-hour virtual session on Wednesday evenings. In addition, there will be two in-person weekend retreats and one virtual weekend.

· I understand that standards and regulations for psychedelic psychotherapy do not currently exist in Canada. I understand this course is a broad introduction to key topics in the field of psychedelic psychotherapy.

· I understand that this is an experiential training program. Weekly virtual sessions will include individual and group reflection and sharing and experiential exercises including meditation and somatic awareness. I will be encouraged to share my experiences with my cohort. The program includes discussions of non-ordinary states of consciousness, psychedelic substances and deep psychological processes which can be triggering to some people. I am willing to undergo training where these states and processes are discussed, demonstrated and practiced.

· This program includes two in-person retreats where I will participate in breathwork sessions led by trained Holotropic Breathwork facilitators. I have read all the material on the Foundations in Psychedelic Psychotherapy Course website about the breathwork experience in this program. I understand that if I am accepted, I will undergo a screening assessment with the breathwork facilitators which will include discussion of my personal history, psychiatric history and medical history to determine if I can safely participate in the breathwork sessions. If it is determined that it not safe to breathe, a full refund minus the application fee will be provided or other accommodations will be made on a case-by-base basis.

I understand that it is strongly recommended that I have a therapist in place for the duration of this program. I understand that breathwork induces a non-ordinary state of consciousness which can bring up intense feelings and content that can be de-stabilizing for a period of days, weeks, months or even longer after the breathwork experiences. By signing up for the course, I understand it is my responsibility to ensure I have adequate supports in place before, during and after the course to aid in my integration process.

· Each member of the cohort, teachers, teaching assistants, breathwork facilitators and course directors will commit to keep what is shared in this training confidential. A list of accepted candidates will be sent out to the entire group in advance of the training. I commit to maintain the confidentiality of my fellow participants in this training program.

· Psychedelic Psychotherapy advances equity and inclusion by providing opportunities and establishing fair practices to support a training environment free from discrimination on the basis of race, religious creed, color, national origin, ancestry, physical disability, mental disability, medical condition, genetic information, marital status, sex, gender, gender identity, gender expression, sexual orientation, or military and veteran status. All trainees are expected to conduct themselves in a manner that contributes positively to a training environment that is supportive and free from discrimination.

I agree to participate in evaluation of the program to be incorporated in future cohorts. This may include surveys, focus groups and interviews.
